Online messages are for general guidance and appointment coordination and do not create a doctor–patient relationship or constitute a diagnosis, an assessment, or a treatment plan. Please do not send clinical records, images or reports through social media.
4. Referral information
Where a clinician refers a patient to this service, the referral and the material sent with it become part of the patient's clinical record and are handled in the same way as any other medical record. Information is shared with the referring clinician as part of answering the referral.
